Qualifications:
Minimum High School Diploma
Must have a valid driver’s license and reliable means of transportation
Spanish language skills strongly preferred
Duties and Responsibilities:
Provide support and assistance to clients and homecare aides
Prioritize duties and be able to manage large caseloads
Responsibly manage clientele files and keep required documentation up to date
Work with case managers and case management units to establish professional relationships and relay relevant information
Help expand the business into new areas through word of mouth and professional networking
Conduct home visits to client’s homes in the area of Chicago & Suburbs
Offer brief training to caregivers about the program as needed
Skills and Abilities:
Applicant should be able to work cooperatively with co-workers and other organizations
Applicant should be able to manage caseload assigned and be able to prioritize tasks
Communicate verbally in Spanish strongly preferred
Applicant should be able to travel within the city of Chicago &/or
Suburbs regularly and travel outside as required
Applicant should have strong written skills to complete documentation and paperwork
Applicant should have knowledge of caregivers and participants’ needs
Familiar with Microsoft office mostly Excel
Role of CAREGIVER:
The Caregiver is responsible for providing direction and assistance with the activities of daily living and personal care necessary for each Client to remain as independent as possible.
Duties and Responsibilities:
Provides Clients with the highest level of assistance with activities of daily living, such as personal care and oral hygiene.
Listens to Client and family concerns and initiates actions and communicates concerns and outcomes to the Supervisor.
Interacts with Clients and their families to build relationships, collect information that assists in meeting their personal needs, and ensure their satisfaction with the personal care services provided.
Assists with ambulation, transferring or transporting of Clients (including pushing Clients in wheelchairs) to the dining rooms, activities and/or various areas.
Communicates any change in Client condition to the Supervisor immediately.
Assists with serving meals and beverages
Observes infection control procedures.
Provides respect, privacy and dignity to all Clients.
Maintains accurate records of each Client’s daily care on the appropriate documents, and in accordance with state and federal regulations.
Comes to work in a clean neat clothes, and consistently presents a professional appearance.
Comes to work as scheduled and consistently demonstrates dependability and punctuality.
Qualifications, Knowledge, Skills, & Abilities:
Experience working in assisted living, home health or long-term care environment preferred.
High school diploma or its equivalent.
Must possess, or be able to obtain current CPR certification.
Must have an adequate understanding of English and be able to speak and write it.
Must be able to work independently, as well as a team Client, with a positive, pro-active attitude.
Follows all policies and procedures of Destiny Healthcare Services.
Must be familiar with the Rights of the Resident, the definition of Resident Abuse, and the reporting protocol of possible Resident Abuse.
Must be a flexible, highly customer service and people oriented individual who demonstrates compassion, patience and understanding, and is willing to do whatever it takes to provide the highest level of service.
Must be a team-oriented individual who enjoys working collaboratively with others, creating a positive and uplifting environment, and communicating very effectively at all levels.
Physical Requirements:
Must be able to manually lift and carry up to 50 lbs. frequently.
Must be able to assist in transferring Clients who weigh in excess of 50 lbs. occasionally.
Must be able to tolerate long periods of sitting, walking, bending and standing.
Must be able to push Clients in wheelchairs frequently for brief periods of time.
