Job Description:
Case manage the patients episode of care
Assessing and re-evaluating the patient’s condition and needs as required
Provide patient care according to patient’s care plan and
Respond and advise on declines in the patient
Teach the patient and/or family members about care and support skills
Communicate regularly with Interdisciplinary Care Team, Office Support Staff, and Other Health Care Providers
Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered
Participate in and share data for Performance Improvement activities to identify opportunities for patient outcome improvement
Supervision of License Practical Nurse
Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life
Qualifications:
Licensed as a Registered Nurse in the state of Illinois
Valid Driver’s License and reliable vehicle with current insurance
Demonstrated ability to work autonomously
Prior Nursing experience in a Home Health, Hospice, Rehab, or Hospital setting is preferred
Experience with Electronic Medical Records (EMR) software is preferred
Comfortable with technology and must have a smart phone or mobile device
Current CPR certification
Strong communication and interpersonal skills
Time management to balance patient care, travel, and documentation
Great customer service
Licensed Physical Therapist
This is a wonderful opportunity for a Licensed Physical Therapist to provide excellent care to our patients. This is a per diem position, with visits Monday thru Friday, and the occasional weekend visit. We are seeking a Home Health Physical Therapist to join our team.

Essential Duties & Responsibilities:
Under a physician’s order, performs the following within scope of practice and federal guidelines:
Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.
Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
Provides direct resident treatment in accordance with an established plan of care.
Instructs patients, families, and caregivers in the care and use of wheelchairs, braces, canes, crutches and other devices.
Maintains patient records.
Devises special tools and treatment for the specific needs of patients.
Conducts home assessments as appropriate to ensure discharge of residents to a safe environment.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
Performs other duties as assigned.
Job Requirements (Education, Experience, Knowledge, Skills & Abilities):
Licensed to practice as physical therapist within the State of Illinois.
Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master’s program in physical therapy approved by the Commission on accreditation in Physical Therapy Education (“CAPTE”) or any successor organization of the CAPTE.
Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
Demonstrates good verbal and written communication and organization skills.
Possesses and maintains current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Licensed Occupational Therapist
This is a wonderful opportunity for a Licensed Occupational Therapist to provide excellent care to our patients. This is a per diem position, with visits Monday thru Friday, and the occasional weekend visit. We are seeking a Home Health Occupational Therapist to join our team!

Under a physician’s orders, screens and evaluates resident/ patient and
develops appropriate care plan.Instructs residents, families and caregivers in the use of adaptive equipment
such as wheelchairs, splints, and aids for eating and dressing.Maintains patient records in an accurate and timely manner.
Devises special tools and treatments for the specific needs of patients.
Completes required forms and documents in accordance with company policy
and state and/or federal regulations.Performs other duties as assigned.
Job Requirements (Education, Experience, Knowledge, Skills & Abilities):
Degree from an accredited Occupational Therapy program.
Minimum of six months occupational therapy experience preferred.
Current and unrestricted Occupational Therapy license in the State of Illinois
Current CPR certification.
Strong organizational and communication skills.
Certified Nursing Assistants
This is a wonderful opportunity for Certified Nursing Assistants (CNAs)/ Home Health Aides (HHA) to join our team.
Job duties include but are not limited to:
Assist in ambulation or exercises
Check vital signs
Provide safe transfer techniques
Assist with medications
Assist in personal hygiene of patient
Communicate with assigned registered nurse regarding condition of patient
Provide personal hygiene and grooming to patients
Complete appropriate records; adhere to professional standards
Job Requirements:
Registered Certified Nursing Assistant (CNA) in the State of Illinois
